Safety & Training Corner
02/15/2006
Hello Everyone,
I am forwarding information about the military surplus meeting that will be in San Francisco October17-19, 2006. It might be of interest to our ALEO membership. I will bring the registration slip to our next meeting.
Contact person:

I have completed the training certificates and will be mailing them tomorrow.
If you have trouble finding a speaker, I have done an earthquake PowerPoint presentation and could easily add the air support role to the earthquake response. Might be an issue with the Katrina report due out this month. Those survivors would still be on the roof, if it was not for Igor Sikorsky's invention. Only a suggestion.

URGENT Safety Issue: Laserdyne Black Opal Flat Panel Displays
There have been two failures in the last month of Laserdyne Black Opal Flat Panel Displays installed in rotorcraft where the glass window faces ruptured and blew glass into the cockpit and on to the flight crew.
These flat panel displays are used in video processing and low visibility image enhancement installations for operations that may include, but are not limited to, aerial surveillance by public use aircraft (law enforcement and Department of Defense, electronic news gathering (ENG), and passenger entertainment/information systems. These flat panel displays may also be installed in fixed wing aircraft.
Installation in the two rotorcraft with the cited failures was by means of field approvals, but these units may also be installed by means of STC's, DER approved data or other approved means.
Dallas Avionics, Inc. is the only known distributor of Laserdyne Flat Panel Displays in North America. The attached list details some of the known recipients of these displays. This list should NOT be distributed outside the FAA.
Current installations appear to be limited to the display of non-essential information but installations portraying information essential to the flight of the aircraft may exist, such as a primary flight display (PFD) or an engine indicating crew alerting system (EICAS).
If ANY flat panel display installations are found, contact the Fort Worth Aircraft Evaluation Group either by telephone at 817-222-5270, or by email at "7-ASW-FTW-AEG/ASW/FAA" for further analysis and disposition.
The attached Special Airworthiness Information Bulletin contains details related to this safety issue.
